The FACT Appliance is designed to reposition the condyle and decompress the joint space to provide pain relief and allow the battered bilaminar zone and shredded supportive connective tissue of the joint time to heal over a period of months. The appliance consists of two acrylic pads with high buccal walls, connected by a Gelb lingual bar. The high buccal walls limit lateral excursive motions of any kind permitting only rotational movement of the mandible. Since the appliance is intended to be worn at all times, the use of buccal walls to stabilize the mandible is favored over lingual walls because the tongue is then free to move a bolus of food up onto the occlusal table of the splint during chewing. In order to properly fabricate the FACT appliance, models, along with a construction bite that accurately represents the condyle in its therapeutic decompressed position, must be provided. Radiographic verification of this position should be accomplished prior to appliance fabrication.
