To move two adjacent molars distally with a removable appliance, maximum anchorage must be utilized while each molar is moved individually. This can be accomplished with one appliance that has two screws aligned next to each other by sequentially turning the screws. First, move the second molar by adjusting the distal most screw. Once you have achieved the desired distalization of the second molar, you may begin adjusting the mesial screw to distalize the first molar. It is important to remember that as the mesial screw is opened one turn, the distal screw needs to be closed one turn. This will keep the second molar in its new position as the first molar is distalized to meet it. Note - When distalizing both first and second molars, it is important to properly instruct the patient about the precise adjustment technique and sequence.
