Carestream
Doctors with a Carestream scanner can send digital impressions directly to SML | Space Maintainers Laboratory through CS Connect.
To send a file:
1. Select the case from the DIS Patient History and click the ‘CS Connect’ button.
2. Log into your CS Connect online account.
3. Select ‘SML | Space Maintainers Laboratory’ under the ‘Choose a laboratory’ tab.
NOTE: If this is the first case being sent to ‘SML | Space Maintainers Laboratory’ enter digital@smlglobal.com in the
email field. SML | Space Maintainers Laboratory will appear in the drop down menu from this point on.
4. Fill-out a form, upload attachments*, enter comments, appliance fabrication instructions, etc. as needed.
NOTE: If desired, download the ‘SML Rx Form’ by clicking on the link found under the ‘Download submission form’
tab. Fill-out and save the SML Rx Form. The SML Rx Form will need to be uploaded with the case by clicking on the
‘Browse’ button located under the ‘Attached files’ tab. (There is no mechanism to edit and save the SML Rx Form
directly from CS Connect.)
5. Submit the case.
